What is Avascular Necrosis?

Avascular Necrosis is a disease where the cells of bone in the ball of the hip joint die due to blood deficiency. This deficiency is caused by blockage to the blood supply. Commonly, the problem starts in the upper end of the femur (thigh bone) and if treatment is delayed, leads to hip joint arthritis. Adults who are 20-60 years old are at risk. In 50 % of cases, both hips are affected. So basically there is dead bone in the ball of the hip. 

When it starts, the patient may not have any symptoms. Gradually, it is manifested by hip pain during walking. In later stages, pain is present even while resting. If not treated on time, it might lead to destruction of the bone, hip joint and disability.

How can it be treated?

Treatment depends on the age of the patient, stage of the disease (early or late), underlying cause and extent of bone damage. Non-operative treatment is successful only in 20% of cases. Some surgical intervention is always required. In the early stage, treatments like osteotomy, bone grafting, core decompression and stem cells are most successful.
In late stages, Hip replacement offers permanent pain relief

Treatment in early stage is possible with core decompression. This consists of relieving the pressure on the blood supply. Its success rate is 60-85 %. Addition of stem cells can improve the success rate of core decompression. Bone-forming stem cells are used to regenerate new bone to replace dead bone.

Treatment in later stages requires Total Hip Replacement. Since most patients are young, it is important to provide a durable hip replacement. Durability is increased by a twin fold approach of preserving patient's own bone and durable materials. Implants that remove less bone are known as short stem hip replacements. Durable materials are Ceramic, metal and special plastics. Total Hip replacement with durable implants can survive for a long period and reduce the need for a redo.
